Sports Physical Night

AllAndrew Lewis and Salem High School athletes must have a yearlyphysical dated after May 1ston the Virginia High School League (VHSL) physical form.  VHSL physical forms can be found here, picked up in the front offices at ALMS and Salem High, or printed off each school’s Athletics website. 

 

We - Andrew Lewis and Salem High -  will conduct a physical night on Wednesday, May 6th,in the sixth-grade wing of Andrew Lewis Middle School.  Andrew Lewis students will be seen from 6:00 to 7:00 PM, and Salem High students from 7:00 to 8:00 PM. Please park on College Avenue in front of Andrew Lewis or in the Central Office parking lot and enter the school through the car rider entrance. The cost is $20, cash only. Please have the physical form filled out completely and signed upon arrival to expedite the process.  Shorts and T-Shirts are appropriate dress for this event.

 

Sports Physical night on Wednesday, May 6th,is a quick, cost-effective way to complete the REQUIRED athletic physical needed for yearly participation in athletics at Andrew Lewis and Salem High.
